The cost is $50.00 per player & is non-refundable.  All players at each level should attend every session.  IN ORDER TO BE CONSIDERED FOR AN A OR AA TEAM, PLAYERS MUST ATTEND AT LEAST ONE TRYOUT.  IF A PLAYER IS NOT ABLE TO ATTEND, HE/SHE MUST CALL WITH A LEGITIMATE EXCUSE.  Please complete the form below, make your check payable to Butler County Blackhawks, and mail by JUNE 1, 2002 to:  Butler County Youth Hockey;  P. We also anticipate House teams at the Mite & Squirt levels.  These teams will be formed in September.  Please return the form so you can be contacted at the appropriate time.
