Midget State Tournament

March 9-14, 2004

 

Tournament Information

 

          There are 6 teams entered in this year’s State Tournament.  All teams will be guaranteed 3 games.  The format will be Round – Robin play and a Championship Game.

 

          We are requesting all teams to send the following information by February 15, 2004.

 

1.                 USA Hockey Team Membership Application Form (2 – T) numbered and signed by District or Associate Registrar.

2.                 USA Hockey Official Player Roster Forms (1 – T) numbered and signed by District or Associate Registrar.

3.                 Copies of Official Score Sheets providing proof that the team has competed in at least

20 games.

4.                 Proof (Official Score Sheets) that each player has participated in at least 10 games.

 

Tournament fee is $800.00 per team.  Please send a check for balance due of $200.00 with

the requested information.  Make checks payable to Buckeye Youth Hockey.
