Awards:        Individual and team awards for 1st and 2nd places

                                                                        MVP awards for each team for all games

By signing this entry form, the team contact  person, on behalf of his/her team, releases the sponsors, organizers, officials, and volunteers of the above named tournament from any liability, injury, or accident which may be incurred by any player, team official, or spectator while participating or travelling to or from the tournament.

 

Only USA Hockey approved teams will be allowed to participate in the tournament.  Please include a copy of your approved USA Hockey Roster and approved USA Hockey Team Membership Application along with your payment with this application.  Also, please email or include a separate typed team roster to be used in the tournament program.  The team roster should include the names and home/away jersey numbers for all players along with the names of all coaches and managers.

 

Please enclose a check made payable to FAHA. (Findlay Amateur Hockey Association)  No applications will be accepted without payment.
